As a new customer you will need to provide our office with a Property Transfer Affidavit (PTA) or the legal documentation obtained showing ownership of service address. This document can be dropped off at our office, Monday-Friday 9:00am - 3:00PM, emailed to multilake@gmail.com, snail mailed or put in our drop box onsite which is available 24/7.
